You tear a pair of linked openings in the fabric of space. Anything that crosses one portal exits the other with momentum and orientation preserved relative to the destination portal.
Rank 1: Hex Portal
The portals are about a hand-span across--big enough to pass an arm, a thrown weapon, or a projectile through. Maximum range is encounter-scale (~60 squares).
Rank 2: Personal Portal
The portals are big enough to walk through. Maximum range is still encounter-scale.
Rank 3: Large Portal
The portals are big enough for a wagon, a horse, or a Large creature. Maximum range extends to visual range (anything you can see clearly).
Rank 4: Wormhole
You can create a true three-dimensional wormhole with effectively unlimited range, provided you have a psychic link to the destination (you have visited it, hold a Psychometry reading of an object from there, or are in mental contact with someone there).
Rank 5: True Wormhole
Your portals can reach anywhere in the universe, even places you have never been, given enough time to focus on a description.
